Designed for Families Who Want to Stay at Home.

We serve individuals who qualify for Medicaid-funded home care - and the family members or trusted loved ones who care for them.
Our focus is simple:
Help people get the care they need at home.
Help caregivers get paid fairly for the work they’re likely already doing.
Who Qualifies for Care?
The program is based on the patient’s eligibility.
An individual may qualify if they:
• Are enrolled in Medicaid
• Are age 65+ (or an eligible disabled adult, depending on the state)
• Need help with activities of daily living such as bathing, dressing, meal preparation, or mobility

Disabled Adults (State-Specific Programs)
In some states, adults (over 18) with physical or cognitive disabilities may choose their own caregiver - including a family member or friend.
Veterans & More
Veterans who require assistance with daily activities, wish to remain safely at home and meet Medicaid criteria.
Who Can Be the Paid Caregiver?
Once the patient qualifies, they can select a trusted person to provide care.
That caregiver may be:

Adult Children
Sons or daughters helping a parent at home.

Other Family Members
Siblings, grandchildren, aunts, uncles, or other trusted relatives.

Close Friends
In many programs, a trusted friend or neighbor who provides consistent care can qualify.
